Amaryl is an oral blood glucose lowering drug of sulphonyl urea class. It is used for the treatment of type 2 diabetes. It works by increasing insulin secretion by pancreatic beta cells. Amaryl is effective as initial therapy.


DESCRIPTION: Amaryl is an oral blood glucose lowering drug of sulphonyl urea class. It is used for the treatment of type 2 diabetes. It works by increasing insulin secretion by pancreatic beta cells. Amaryl is effective as initial therapy. If the monotherapy of amaryl is not giving any result, we can give a combination of amary and metformin. It will give a synergestic effect. Amaryl is available in the dosage forms of 1mg, 2mg, 3mg and 4mg tablets.

INDICATION: amaryl is indicated as an adjunct to diet and exercise to improve glycemic control in patients with type 2 diabetes mellitus.

MECHANISM OF ACTION: The primary mechanism of action of amaryl is increasing the insulin secretion from the pancreatic beta cells. Glimipride, the content of amaryl can increases the sensitivity of peripheral tissues to insulin.

DRUG INTERACTION: The hypoglycemic activity of glimipride is potentiated by other drugs like non steroidal anti-inflammatory agent, fluoxetine, clarithromycin and disopyramide.  When amaryl is used with oral miconazole, severe hypoglycemia may occur. Certain drugs like thiazides and other diuretics, corticosteroids, thyroid products, phenothiazine may cause hyperglycemia leading to loss of control. Glimipride will increases the serum level of ciclosporins. With fluconazole, it may increases the blood level of glimipride.

CONTRA INDICATIONS: Amaryl is contraindicated in the patients with diabetic ketoacidosis with or without coma.

SIDE EFFECTS: Vomiting, gastro intestinal pain, diarrhea, urticaria, purities, morbilliform, maculopapular eruption,leucopenia, agranulocytosis, thrombocytopenia, hemolytic anemia, aplastic anemia and pancytopenia, hyponatremia, blurred vision, and jaundice, prophyria cutanea tarda, photo sensitivity reaction and allergic vasculitis also reported with suphonyl urea including gimipride. In an isolated instances liver impairment as well as hepatitis which may lead to liver failure also has been reported with sulphonyl urea. Amaryl may also produce severe skin rash, pale skin, easy bruising or bleeding, fever, unusual weakness and trouble breath.

Special precaution should take when it is given to the patient with increased risk of cardiovascular mortality, elderly, hepatic and renal impairment.  Symptoms of increased secretion of anti diuretic hormone in patients with congestive heart failure or hepatic cirrhosis. Monitor the blood glucose concentration in pregnancy and lactation. Treatments of patients with glucose-6 phosphate dehydrogenase deficiency with sulphonyl urea may lead to hemolytic anemia.
